We recommend housing estates that have not conducted our “Be a Responsible Pet Owner” seminar before to choose topic (1). If time allows, housing estates can add other *optional topics.

Housing estates that have conducted our “Be a Responsible Pet Owner” seminar before can choose any *optional topics.

We will give out relevant souvenirs, promotional posters and leaflets to your housing estates during our seminar to promote proper control of pets.

I have read and understood the rules below.

As the seminar is provided free of charge, please do NOT charge participants any fees.
AFCD will arrange the seminar according to a first come, first served basis.
To ensure efficient use of resources, AFCD will generally confirm a seminar booking if there are at least ten participants signing up for the proposed seminar one week before it is scheduled to be held.
If necessary, the date and time of the seminar might be changed.
If there are any changes, AFCD will contact the applicant and confirm with him / her.
The date and time of the seminar must be confirmed by AFCD.
The duration of the seminar will depend on the topics chosen.
The seminar will be conducted in the form of a PowerPoint Presentation and display of videos.
Please provide one parking space, two wireless microphones, a computer that can show PowerPoint and videos, a projector, a screen, sound system, a table and three chairs for the seminar.
AFCD’s final decisions are subject to evaluation of the situation and information provided by applicants.

The information provided by the applicant will be used for purposes related to the organisation of educational seminar by AFCD. The provision of personal data is voluntary but we may not be able to process the booking if the information is not provided.
AFCD may give some of the information to other parties authorised by law to receive it.
Under the Personal Data (Privacy) Ordinance, the applicant has the right of access and correction with respect to the personal data on this Seminar Booking Form.
